Understanding Istihadah: Prayer, Fasting, Purification and Marital Relations

Assalamu alaikum wa rahmatullahi wa barakatuh.

What is the Islamic ruling concerning istihāḍah (non-menstrual bleeding) in women?

Can a woman experiencing istihāḍah perform her prayers and voluntary fasts? Is her husband permitted to have marital relations with her while she is experiencing this bleeding? And how should she maintain her purification?

ANSWER ❗️

Wa alaikumussalam wa rahmatullahi wa barakatuh.

Istihāḍah is abnormal or non-menstrual bleeding. It is different from ḥayḍ (menstruation) and therefore does not carry the same rulings as menstruation.

A woman experiencing istihāḍah remains in the ruling of a woman who is able to worship. She should therefore continue praying and fasting, and marital relations remain permissible.

1. Can a Woman Experiencing Istihāḍah Pray?

Yes.

The Prophet ﷺ explained the distinction between menstruation and istihāḍah when Fāṭimah bint Abī Ḥubaysh (may Allah be pleased with her) asked about her prolonged bleeding. He said:

«إِنَّمَا ذَلِكِ عِرْقٌ وَلَيْسَ بِالْحَيْضَةِ، فَإِذَا أَقْبَلَتِ الْحَيْضَةُ فَدَعِي الصَّلَاةَ، وَإِذَا أَدْبَرَتْ فَاغْسِلِي عَنْكِ الدَّمَ وَصَلِّي»

Transliteration:
Innamā dhāliki ʿirq, wa laysa bil-ḥayḍah. Fa-idhā aqbalatil-ḥayḍatu fadaʿī aṣ-ṣalāh, wa idhā adbarat faghhsilī ʿankid-dama wa ṣallī.

Translation:
“It is only a blood vessel and is not menstruation. So when menstruation comes, refrain from prayer; and when it goes away, wash the blood from yourself and pray.”

Therefore, once a woman knows that the bleeding is istihāḍah rather than menstruation, she does not abandon her prayers because of it.

2. Can She Fast?

Yes.

A woman experiencing istihāḍah may perform both obligatory and voluntary fasts. Istihāḍah does not have the same ruling as menstruation and does not prevent fasting.

If the bleeding occurs during Ramadan, she continues fasting unless there is another legitimate reason preventing her from fasting.

3. Can Her Husband Have Marital Relations With Her?

Yes.

Because istihāḍah is not menstruation, the prohibition concerning intercourse during menstruation does not apply to a woman experiencing istihāḍah.

Allah says regarding menstruation:

قُلْ هُوَ أَذًى فَاعْتَزِلُوا النِّسَاءَ فِي الْمَحِيضِ

Transliteration:
Qul huwa adhan faʿtazilū an-nisāʾa fil-maḥīḍ.

Translation:
“Say, ‘It is harm, so keep away from women during menstruation.’”

Qur’an 2:222

The prohibition in this verse concerns menstruation, not istihāḍah.

Therefore, if the bleeding is genuinely istihāḍah, marital relations are permissible.

4. How Does a Woman With Istihāḍah Maintain Purification?

She should clean the blood from the affected area and take reasonable measures to prevent the blood from spreading onto her clothes or body.

She then performs wuḍūʾ and prays.

There are specific details in the fiqh books concerning how frequently a woman with continuous bleeding should renew her wuḍūʾ. One well-known position is that she performs wuḍūʾ after the prayer time begins and then performs the prayer.

However, the detailed ruling differs among the schools of Islamic jurisprudence, particularly for women whose bleeding is continuous or intermittent.

She should therefore follow the established practice of the school of fiqh she follows or consult a qualified scholar if her bleeding pattern is complicated.

5. She Should Distinguish Menstruation From Istihāḍah

This distinction is very important.

Not every bleeding experienced by a woman is automatically istihāḍah, and not every bleeding outside her usual pattern is automatically menstruation.

The ruling may depend on factors such as:

  • her normal menstrual cycle;
  • the duration of the bleeding;
  • whether there was a clearly established period of purity;
  • the characteristics of the blood;
  • and the relevant fiqh principles.

Therefore, a woman who has irregular or prolonged bleeding should not simply label every episode as istihāḍah without considering these factors.

Conclusion

A woman experiencing genuine istihāḍah:

  • may and must pray;
  • may perform obligatory and voluntary fasts;
  • may engage in marital relations;
  • should clean the affected area and take reasonable measures to contain the bleeding;
  • and should maintain her purification according to the applicable rules of wuḍūʾ.

The most important point is to distinguish istihāḍah from genuine menstruation, because the two have different Islamic rulings.
